GLP-1, Ozempic, and Wegovy: Your HSA/FSA Guide for 2026

DoctorNoted Team·6 min

GLP-1 medications — Ozempic, Wegovy, Mounjaro, Zepbound — have transformed weight loss and diabetes management. They've also created confusion around what's HSA/FSA-eligible.

Here's what's actually covered, and how to maximize your account dollars when you're on a GLP-1.

Are GLP-1 Medications HSA/FSA Eligible?

Yes — when prescribed. Prescription medications are categorically HSA/FSA eligible under IRS Publication 502, full stop. If your doctor prescribed Ozempic, Wegovy, Mounjaro, or Zepbound, the prescription cost (copay, coinsurance, or full cost if uninsured) is reimbursable.

This applies whether the prescription is for type 2 diabetes (Ozempic, Mounjaro) or weight loss (Wegovy, Zepbound). The IRS doesn't distinguish between FDA-approved indications when the medication is prescribed.

What About the Exercise Your Doctor Recommended Alongside?

Most prescribers recommend exercise alongside GLP-1 therapy to preserve muscle mass and improve metabolic outcomes. The exercise is also reimbursable, but only with a separate Letter of Medical Necessity.

Your obesity, type 2 diabetes, or pre-diabetes diagnosis is what qualifies you. The LMN documents that gym/training/programs are medically necessary for that condition. With it on file, your gym, personal training, structured weight loss programs, and home equipment all become reimbursable.

What's Reimbursable Alongside GLP-1 Therapy

  • Gym/health club membership
  • Personal training (especially resistance training to preserve muscle mass)
  • Structured weight loss programs (Wondr, Noom, etc.)
  • Home equipment for cardio + strength
  • Body composition monitors
  • Continuous glucose monitors (CGMs)
  • Heart rate monitors

A Common Pitfall

Some patients assume their GLP-1 prescription itself covers exercise expenses. It doesn't. The medication is prescribed for the condition; the exercise needs its own LMN documenting medical necessity for the same (or related) condition.

The good news: getting the LMN is straightforward when you already have a documented diagnosis driving your GLP-1 prescription.

Our Recommendation

If you're on a GLP-1 medication and your doctor has recommended exercise:

  1. Verify your prescription is being reimbursed through your HSA/FSA — most admins handle prescriptions automatically.
  2. Get a Letter of Medical Necessity for exercise — separately documents your gym/training as part of treatment.
  3. Track receipts for both — prescription receipts plus fitness expense receipts.

Combined, GLP-1 patients often see $3,000–$8,000+ in annual reimbursable expenses.
